Providence Home Health Clinical Manager - Home Health Snohomish in Everett, Washington
Providence is calling a Home Health Clinical Manager (1.0 FTE, Day Shift) to the Home Health Snohomish at Providence Hospice & Home Care of Snohomish County in Everett, Washington.
This position is a licensed RN, PT, OT or SLP with the skills and abilities to perform home health staff RN or Rehab duties. The Home Health Clinical Manager serves as a communication hub responsible for coordinating the multidisciplinary home health services for the clinical team. This position acts as a liaison between Access Data Entry, Scheduling, Quality and clinical Staff.
In this position you will:
Provides clinical oversight to multidisciplinary team members for designated team with the support of a discipline supervisor as required for discipline-specific input
Interviews and hires caregivers; as needed identifies performance concerns and gives timely feedback to direct reports; develops and monitors improvement plans and delivers coaching and/or corrective actions. Works closely with HR to investigate performance issues and terminate employment when necessary
Assigns and/or provides consultative support for referrals as appropriate to clinicians based on clinical skill set and competency for a multidisciplinary team of clinical employees
Provides oversight and timely review of the Home Health caregivers OASIS/Plans of Care/documentation for delivery of home health services with goal of optimal patient outcomes and appropriate reimbursement by utilizing available resources. Assists with the accurate and timely implementation of corrections to clinical records
Provides vertical support between Home Health clinical staff and leaders, focusing these support efforts on fulfilling ministry and service line strategic goals including compiling data and/or reports utilizing workflow and financial planning information
Required qualifications for this position include:
Bachelor's Degree in Health care related field
Current and in good standing RN, PT, OT or SLP license in Washington State
Current Driver’s license and have access to daily use of an insured, reliable vehicle
Current Provider CPR Certification
Certified OASIS Specialist-Clinical certification is required or must be earned within one year from date of hire
Three (3) years experience in the nursing or rehab field.
