Providence is calling a Home Health Aide (CNA) (Hospice Snohomish, Days, 1.0 FTE) to SCS Hospice in Everett, Washington. This is patient care supportive work involving personal care and related services for patients according to the physician plan of treatment under the direction and supervision of a registered nurse, physical therapist, speech language pathologist, or occupational therapist. The work is evaluated through direct observation, consultation and written documents.

In this position you will:

  • Performs appropriate and safe personal hygiene and grooming including:

  • Bed bath,

  • Sponge, tub or shower bath,

  • Shampoo, sink, tub or bed,

  • Nail and skin care,

  • Oral hygiene,

  • Toileting and elimination.

  • Teaches these techniques to patient/patient family when directed in plan of care

  • Transfers patients safely, using both good transfer techniques for patient safety and good body mechanics for caregiver safety.

  • Maintains a clean, safe and healthy environment. Communicates safety hazards and environmental health hazards to primary clinician and/or team members.

  • Demonstrates knowledge of care plans as developed by primary clinician and team.

  • Prioritizes patient’s needs and meet care plan objectives.

  • Reports and documents patient status and the care (or service) provided in relation to the individual care plan.

  • Maintains accurate schedule for patient care.

  • Arrive at patient home in a timely manner. Communicates problems with schedule to Home Health Manager/Supervisor as soon as possible.

  • Communicates with patients (and patients’ families), with primary clinician and team members; reporting accurately, and in a timely manner, changes in patient status.

Qualifications:

Required qualifications for this position include:

  • H.S. Diploma or GED

  • Completion of a certified nursing assistant course

  • 1 year of experience as an aide in an acute or rehabilitative facility.

  • Current Washington State Nursing Assistant Certification.

  • Valid Washington State driver’s license.

  • Proof of vehicle insurance.

  • Healthcare Provider BLS/CPR certification required.
