Providence Health & Services Discharge Planner - Social Services in Everett, Washington

Providence is calling a Discharge Planner to Providence Regional Medical Center Everett in Everett, WA.

We are seeking a detail-oriented licensed social worker to assure patients are safely discharged. This is a day shift position (.8 FTE / 32 hours a week) that also includes a weekend rotation.

In this position you will:

  • Plan and coordinate activities related to discharge planning

  • Develop and implement a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care

  • Be responsible for the collaboration with medical staff, patients, family, community agencies, or facilities involved in patient’s care, and the health care team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes

Qualifications:

Required qualifications for this position include:

  • Masters Degree - Social Work, Counseling or Bachelors degree in Nursing

  • Current licensure/certification for education

  • Prior discharge planning experience with medical patients in-patient/outpatient settings

  • Current Washington State Registered Nurse (RN) licensure OR Current Washington State Social Work licensure; Current Agency Affiliated Counselor, Social Worker Advanced License, Social Worker Associate, Licensed Mental Health Counselor or Associate; Licensed Advanced Social Worker or Associate; Licensed Independent Clinical Social Worker or Associate

Preferred qualifications for this position include:

  • Current Washington State Registered Nurse (RN) licensure

  • Two to five (2-5) years’ experience discharge planning for medical patients in an inpatient or outpatient setting

  • Diverse clinical background

  • Prior experience in a hospital setting doing discharge planning

  • Proficient with conflict management

  • Prior experience using Electronic Medical Records (EMR) such as EPIC, and Microsoft Office Suite

  • Prior experience with Quality Improvement
