Providence Community Health Worker in Everett, Washington

100% remote, but MUST live within North Sound Region. You will need to have familiarity with the community you will serve.

Providence is calling a Community Health Worker (1.0 FTE, Day Shift) to our Administration Department at Providence Medical Group in Everett, WA. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible to all - especially those who are poor and vulnerable.

The Community Health Worker (CHW) reports to the Manager of Population Health, and must be comfortable working with individuals who may have medical and/or behavioral health conditions and/or substance use disorder. The CHW works directly with patients and their families in need of COVID-specific resources as well as identify and address barriers to health through screenings (i.e., gaps in care, food, housing, support, transportation).

  • Develops and maintains a positive and professional relationship with community stakeholders that promotes support for continued involvement and cooperation in Population Health projects

  • Actively seek opportunities for outreach with a focus on hard to reach populations and our identified “high-need” communities.

  • Plan, coordinate and conduct outreach events and presentations about available resources, services and health and social programs (i.e.,Medicaid and SNAP, etc.) to local community based organizations and community residents.

  • Maintain and strengthen relationships with key community stakeholders

  • Refers clients/patients to the appropriate programs in the community that meet the needs identified in the screenings

  • Document all outreach events and direct services provided in the electronic medical record, including maintenance of logs and sign-in sheets

Qualifications:

Required qualifications for this position include:

  • High School Diploma or GED

  • Valid Driver’s license and auto Insurance

  • Three (3) years Bilingual in Spanish, Arabic, Mandarin/Chinese, or Vietnamese

Preferred qualifications for this position include:

  • Associate's Degree or Bachelor's Degree in Health or Social work related field

  • One (1) year Related Community Health or Outreach experience.

  • CHW or Clinical Certification or Licensure. Received CHW certification or clinical/medical certification or license from country of origin
