Providence Health & Services Case Management Coordinator - Social Services in Everett, Washington
Providence is calling a Case Management Coordinator Social Services (1.0 FTE, Days) to work in Social Services to Providence Health & Services in Everett, WA.
We are seeking a Case Management Coordinator Social Services who will be responsible for acting as a resource for assigned case management department staff in problem-solving, serves as a role model, and provides limited supervision of department staff, functions, and processes. In addition, the Case Management Coordinator Social Services provides case management services to a select group of assigned patients in accordance with department standards, policies, and procedures
In this position you will:
Work closely and collaboratively with Manager and Director to develop department objectives and strategies to maximize staff efficiencies and operations that support organizational strategies and initiatives
Collaborates with the manager and director in employee performance evaluation.
Identifies and collaborates clinical and operational issues that impact the provision of services with manager and director
Assist and develop a successful and cohesive team, with high level of productivity and accuracy to achieve performance metrics
Assures appropriate and timely response to issues that impact the provision of services
Promote positive working environment for staff, understanding the importance of team-based care in service delivery
Works well with various groups of people, teams and organizations functions
Adjusts personal communication style in order to collaborate and influence with respect
Stabilizes groups by listening, staying calm, and unifying people with their words and actions
Oversight of the day-to-day operations of the team to ensure regulatory standards are met
Identifies clinical an operational issues and addresses them in a timely and appropriate manner and/or escalates to manager
Oversee administrative functions of the team, monitoring department productivity, reviewing department policies and procedures, and monitoring individual staff workloads/assignments
Assist with case management workflows and standard work processes, as well as assist with department policies and procedures
Perform audits of assessments, care plans and service notes to verify cases are properly managed and client coordination activities are occurring and appropriately documented
Proactively monitor defined metrics to ensure quality service delivery and compliance with program standards
Act as a resource to answer questions and assist peers and management with delegated tasks and projects
Required qualifications for this position include:
Three (3) years discharge planning and case management in an acute care setting
Working knowledge of MS Office Suite, including but not limited to Word, Outlook, and Excel
Licensed Mental Health Counselor
Licensed Independent Clinical Social Worker
WA State DOH licensure
Preferred qualifications for this position include:
Coursework/Training - Crucial Accountability Training
Coursework/Training - Emotional Intelligence Training
Case Manager Certification or American Case Management Certification
