Providence Health & Services RN - Care Coordinator, Colville, WA in Colville, Washington
Providence is calling a Registered Nurse (RN) - Care Coordinator (Support Services, Days, 1.0 FTE) to Providence Medical Group in Colville, Washington. This RN wil provide patient care to our Stevens County patients who are undergoing treatment for opioid management and/or drug addiction. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Assess the physiological, psychosocial, spiritual and cultural needs of the patient/family.
Accurately complete home health assessments utilizing observation and interview techniques as required by program guidelines.
Identify the need for HHA, MSW, Pharmacy, OT, PT, RD, or SLP services.
Review all medications the patient is currently using in order to identify any potential adverse effects and drug reactions, including ineffective drug therapy, significant side effects, significant drug interactions, duplicate drug therapy, and noncompliance with drug therapy.
Correlate assessment findings with appropriate interventions and follow up.
Mutually establish a Plan of Care with the patient/family and physician.
Required qualifications for this position include:
Graduation from an accredited school of nursing
Currently licensed as a Registered Nurse (RN) in the State of Washington
Current Basic Life Saving (BLS) certification through the American Heart Association (AHA) within 90 days of hire
Preferred qualifications for this position include:
- Experience working with patients on opioid management and/or drug addiction.
