Providence Health & Services Patient Navigator - Support Services in Colville, Washington
Providence is calling a Patient Navigator (Support Services, Days, 1.0 FTE - 40 hours week) to Providence Medical Group in Colville, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this brand new position you will:
Coordinate services and guides the poor and the vulnerable across the continuum of care to help achieve the best possible care and services.
Describe the services available and has a solid understanding of various Providence-provided and community-based resources and can advise patients on how to best utilize the resources available.
Work as part of an integrated team that includes providers, social workers, and a broad scope of community resource leaders to effectively address the needs of each patient in a collaborative, patient-centered environment.
Perform all duties in a manner which promotes team concept and reflects the mission and values of Providence Health & Services.
Required qualifications for this position include:
High School diploma or equivalent
Current American Heart Association Basic Life Support (AHA-BLS) for Healthcare Provider or obtained within 90 days of hire
Current Washington or Idaho driver’s license
An excellent communicator with the ability to build bridges toward the common mission
Experience working with the poor and vulnerable
Excellent computer skills required with proficiency in Microsoft Outlook, Word, Excel, and PowerPoint
Ability to effectively communicate to patients, customers, co-worker or leaders via written or verbal communication
Preferred qualifications for this position include:
Medical certification or licensure
Healthcare and/or customer-service role experience
Demonstrated knowledge of basic medical terminology
