Providence is calling a Staff Nurse Home Health (RN) (Full-time/Day-shift) at Providence Saint Joseph Home Health department. This position will travel to see patients in the Burbank/San Fernando Valley areas.

We are seeking a Staff Nurse Home Health (RN) a professional nurse who will apply the nursing process to develop and execute an age-appropriate plan of care for assigned home care/hospice patients by coordinating the interdisciplinary team to implement and/or update the patient plan of care according to assessment findings.
In this position you will:
Implement and maintain established department, unit, administrative, and clinical policies/procedures for patient care, safety, and infection control
Participate in improving organizational performance activities as assigned
Conduct initial and ongoing patient assessment with attention to the patient's physical, emotional, psychosocial, functional, home environment, and spiritual needs
Develop and implement individualized plan of care for each patient
Develop and teach individualized patient education according to assessed learning needs
Provide care to patients in the home and consults with other specialists as indicated
Assess each patient's clinical condition per unit/dept standards and report changes as appropriate
Administer prescribed medications and treatments according to physician orders and/or approved protocols
Collaborate and communicate with other personnel and physicians to optimize patient care according to the plan of care
Coordinate and provide the necessary equipment/supplies needed for patient care
Document in a clear, concise, and organized manner to ensure the accuracy of the medical record to include updating the patient plan of care and medication profile
Provide direction to team members to coordinate care of assigned patients and problem-solve patient care issues
Assist patient/family in discharge planning and continuity of care after home health, i.e., how/where to obtain supplies, services from community resources, outpatient clinics
Orient and educate new employees and students as assigned
Prepare patients for scheduled procedures and assists physicians as needed
Demonstrate sensitivity to each patient's comfort, safety, and privacy needs
Activate the chain of command and to follow through on complaints, incidents, ethical concerns, or unusual occurrences
Participate in meetings, case conferences, educational events, license/certification renewals, and professional associations as appropriate to nursing specialty
Required qualifications:
MOAB I required within 30 to 150 days of hire
Current California Licensed Registered Nurse
Current BLS HCP Basic Life Support for Healthcare Providers American Heart Association (AHA) or American Red Cross (ARC) - CPR/AED for Professional Rescuer accepted on hire until card expiration date
Current California Driver License
1 year professional nursing experience
Experience in cardiac care (for Staff on the Cardiac team
1 year experience in Maternal and Child Nursing (for Staff on the Maternal and Child team)
