Intake Business Coordinator - Home Health
Location: Anaheim, CA
Work Schedule: Full Time - 80 Biweekly Hours
Shift: 8-hour, Days (Weekends required)
This position is shared services supporting Orange and Los Angeles counties and is based out of Anaheim, CA.
The Intake Business Coordinator is responsible for timely conversion of referrals into admissions, including but not limited to:
Review of documentation to ensure all information is complete and provided for clinicians
Accurate data entry to facilitate the new admissions process across all departments of our infusion pharmacy
Thorough documentation of all communication/conversations related to care coordination, timeframes and outcomes
Proper prioritization and escalation of urgent referrals
Support the process of scheduling clinical staff as needed for infusion therapies
Establish and maintain relationships with referral sources through excellent customer service
Required qualifications:
- 2 years experience in data entry.
