Providence Health & Services RN Care Coordinator Supervisor in Beaverton, Oregon
Providence is calling a RN Care Coordinator Supervisor to Providence Health & Services in Beaverton, OR.
The RN Care Coordinator Supervisor is responsible for providing day to day oversight for the general operations of the Care Management and Medical Review teams within Quality Medical Management (QMM) and works in cooperation with the Manager to provide leadership and guidance for the QMM staff. In addition to supervisory responsibilities, duties include all functions of the RN Care Coordinator, providing case management or medical review, including coordination and assistance to patients with complex health needs or the potential for poor health outcomes.
In this position you will have the following responsibilities:
Oversee the daily activities of staff, including hiring and performance management.
Perform the six essential activities of case management: assessment, coordination, planning, monitoring, implementation and evaluation in multiple environments.
Perform case management of chronic and catastrophic cases to effectively manage services, benefits, costs and quality following policies and procedures of the Utilization Management Program.
Provide case management for specialty programs as required by commercial and government contracts or product design.
Develop and maintain positive relationships with providers and facilities to facilitate case management; maintain knowledge in subject matter and enhance Health Plan expertise in the area of case management.
Track and report case management activities, savings and outcomes as appropriate for the individual program and as required, supporting the Utilization Management Program.
Required qualifications for this position include:
Bachelor’s degree in nursing or health care. Applicable work experience may be substituted for some education.
Graduation from an accredited school of nursing or graduation from an accredited equivalent clinical program.
5 years of clinical experience.
Current licensed driver.
Currently licensed as a registered nurse in the state of Oregon.
Demonstrated leadership ability.
Ability to make independent decisions.
Effective verbal and written communication and problem solving skills.
Demonstrated ability to coach and teach to enhance team development.
Demonstrated ability to understand, adapt and respond effectively to a constantly changing environment.
Strong organizational, analytical, and leadership skills.
Excellent oral and written communication skills.
Ability to work in a dynamic environment, handle change and remain flexible.
Ability to conduct oneself in a professional manner, both orally and in writing.
Ability to work independently, to be self-directed, and to function within a team environment.
Demonstrated creativity and flexibility in problem solving and development of alternative approaches to the traditional delivery of health care.
Ability to manage multiple projects and staff simultaneously to achieve department and company strategic goals.
Experience in working with physicians in collaboration and management of patient care.
Demonstrated experience in project management, educational program development and implementation, and personal computers.
Preferred qualifications for this position include:
Previous supervisory or lead experience.
Experience in Care Management and/or Medical Management in a Managed Care setting.
