Providence Health & Services RN Access Coordinator - Home Health in Anchorage, Alaska


Providence is calling a Registered Nurse (RN) Access Coordinator (Administration, Day Shift, 1.0 FTE) to Providence In Home Services in Anchorage, Alaska.

The RN Access Coordinator will facilitate the intake processes for Providence Home Health and Hospice.

In this position you will:

  • Coordinate and facilitate program essentials and assesse potential clients for the appropriate and timely utilization of home health and hospice services for all referrals.

  • Communicate patient status to appropriate clinical team to include: situation, background, assessment, and recommendations (SBAR).

  • Utilize tools adapted through quality and safety initiatives when communicating patient status.

  • Have on-going responsibility for coordination with insurance companies and other payers as required for prior authorizations and reimbursement.

  • Serve as a conduit for accurate and timely information to key people on potential or current clients; document information appropriately.

  • Provide information and consultations to clients, potential clients, families, physicians, hospital staff, and to the community in general to assess needs, identify options, and guide the arrangements for home health and hospice services.


Required qualifications for this position include:

  • Graduate of an accredited/approved school of Nursing

  • Basic Life Support (BLS) certification

  • Current Alaska Registered Nurse license

  • Alaska State Driver’s license

  • Computer skills

Preferred qualifications for this position include:

  • 4 years clinical experience

  • 2 years in Medicare certified home health, Medicare certified hospice or community health nursing

  • Knowledge of Home Health and Hospice regulations

  • Working knowledge of criteria for reimbursement from Medicare, Medicaid, insurance, and other funding sources
