Providence Health & Services Medical Social Worker (MSW) - Senior Care in Anchorage, Alaska
Providence is calling a Social Worker (MSW) (Senior Care Clinic, 1.0 FTE, Day shift) to Providence Physician Services in Anchorage, Alaska.
In this position you will:
Provide professional social work services, counseling and groups to improve or maintain the social, emotional, functional and physical health status of the patient.
Work in collaboration with patient and/or family, physician, nurses, therapists, other staff and community resources, to access, plan, implement and evaluate appropriate treatment.
Responsible for determining and assisting with the resolution of situational needs of patients and families.
Collaborate with interdisciplinary team in the process of assessment and evaluation.
Required qualifications for this position include:
Master's degree in Social Work (MSW)
Alaska Licensed Masters of Social Work (LMSW) or Alaska Licensed Clinical Social Worker (LCSW)
Preferred qualifications for this position include:
2 years of acute care Medical Social Worker experience
Care Coordination license
Knowledge of community resources
