Providence Health & Services Faculty Pharmacist - Family Medicine Center in Anchorage, Alaska
Providence is calling a Faculty Pharmacist (AK Family Medicine Residency, Day Shift, 1.0 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. In this position you will:
Provide collaborative mediation therapy management (MTM) for Warfarin patients at PFMC. This involves organizing and supervising the care plan for Warfarin patients, collection and analyzing of the PT/INR, adjusting Warfarin doses, and ensuring tracking and follow-up. This work will be under the authority and supervision of the PFMC Medical Director and in collaboration with the lab personnel and physicians. Educate patients on Warfarin Management.
Participate in group visits of patients to provided education and medication review.
With other faculty, develop a curriculum in pharmaceuticals, including goals, educational strategies, competencies and evaluation, to improve residents’ knowledge and practices in prescribing. Provide didactic scenarios, one-on-one teaching, and participate in case rounds to achieve the goals of the curriculum.
Assist Medical Director in developing quality assessment and improvement plans for use of medications at PFMC. Provide feedback to physicians individually and in groups to improve patient care. Monitor medication use and prescribing practices clinic-wide.
Act as liaison with Providence Alaska Medical Center (PAMC) pharmacy department, maintaining communication and current knowledge of PAMC’s policies and issues. Support PAMC’s PharmD residency program, encouraging residents to rotate at PFMC for ambulatory training. Support clinic management and nursing staff in maintaining medications at PFMC, including vaccines, and injectables.
Required qualifications for this position include:
Graduate of an accredited college of pharmacy
Doctor of Pharmacy degree
Current Alaska Pharmacy license
Preferred qualifications for this position include:
Experience in ambulatory care, inpatient care, Warfarin management and/or teaching, MTM collaboration
Working knowledge of database software
