Providence Health & Services Licensed Vocational Nurse - Hospice Intake ( Per Diem - Day Shift ) in Torrance, California
Providence is calling a per diem, variable shift Intake Licensed Practical Nurse ( LPN ) / Licensed Vocational Nurse ( LVN ) to Providence TrinityCare Hospice in Torrance, California
This is an office-based position.
In this position you will:
Act as a contact for information and coordination of the initial Providence TrinityCare Hospice services from physicians, insurance companies, discharge planners and other outside referral sources.
Provide access for new patients to Providence TrinityCare Hospice services.
Required qualifications for this position include:
Graduate of an accredited practical or vocational nursing program.
Currently licensed as a LPN / LVN in the State of California.
Possesses and maintains current BLS Certification.
Minimum of one (1) year of nursing related work experience, demonstrating a working knowledge of clinical care practices and procedures.
Valid Driver's license with an automobile that is in good working order and insured in accordance with State and/or LCMHS / Providence TrinityCare Hospice requirements.
Must be able to work weekend rotation.
- Community health/hospice, medical/surgical or case management experience.
