Providence is calling a Full Time, Day Shift Clinical Social Worker to Providence TrinityCare based in Torrance, CA.
This position provides community-based palliative care to the aging Japanese population in the LA and South Bay areas.

In this position you will:
Participate in theactivities of the interdisciplinary Palliative Care Team; and in thedevelopment and review of appropriate protocols, facilitates formal andinformal meetings regarding patient and family communication and decisionmaking processes that support appropriate referral and consistent care, worksin collaboration with physicians, hospital,home health and hospice health care teams to provide Palliative Care services inpatient, out-patient and in home settings to seriously ill patients and theirfamilies.
Engage inindependent decision-making about psychosocial needs, provides psychosocialservices to individuals and their families, and provides formal and informalmeetings regarding patient and family communication and decision making.
Obtain a psychosocial history,including chart review and informationgathered from the patient and family, and conducts psychosocial palliative careneeds assessments.
Provide direct patient care as well asconsultative services to assure the delivery of state-of- the-art palliativecare. In addition to these clinical duties, the Palliative Care MSW isresponsible for contributing social work expertise to staff education, datacollection, and quality improvement activities as they relate to inpatient,outpatient and home palliative services.
Required qualifications for this position include:
Graduate of a Master’s program in social work accredited by the Council on Social Work Education.
Minimum 3-year’s of inpatient, outpatient, or home-based social work experience.
Demonstrates evidence of experience in palliative care settings, such as hospice.
Demonstrates good verbal and written communication skills and organization skills.
Bilingual in English/Japanese
Demonstrates knowledge of the psychosocial aspects of disease progression in the aging Japanese community
Working knowledge of computer applications, such as Microsoft Office
Preferred qualifications for this position include:
Advanced training in palliative care and geriatric social work
Demonstrates working knowledge of computer applications, i.e. Microsoft Office and e-mail.
