Providence Health & Services Case Manager Registered Nurse ( Full Time - Day Shift ) in Torrance, California
Providence is calling a full time, day shift Case Manager to Providence Little Company of Mary Medical Center in Torrance, CA.
In this position you will:
Perform the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client’s care.
Conducting a comprehensive assessment of the client’s health and psychosocial needs, including health literacy status and deficits, and develops a case management plan collaboratively with the client and family or caregiver.
Educating the client, the family or caregiver, and members of the health care delivery team about treatment options, community resources, insurance benefits, psychosocial concerns, case management, etc., so that timely and informed decisions can be made.
Advocating for both the client and the payer to facilitate positive outcomes for the client, the health care team, and the payer.
Assisting the client in the safe transitioning of care to the next most appropriate level.
Required qualifications for this position include:
Current California Registered Nurse ( RN ) license
2-3 year’s acute hospital experience, including one year of supervisory experience.
Strong knowledge base or experience in specialty area assigned
Preferred qualifications for this position include:
Bachelor's Degree in Nursing ( BSN )
Master's Degree in health care related field
Case Management experience
Critical Care or Medical/Surgical experience
