Providence Health & Services Social Worker MSW - Home Health in Spokane Valley, Washington
Providence is calling a Social Worker (MSW) (Home Health, Day Shift, 1.0FTE, 40hrs/wk) to Providence VNA Home Health in the Greater Spokane County Area, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Enable patients and their families to recover or maintain optimal levels of physical, mental, and psychosocial well being.
Use your exceptional communication skills to help patients and families deal with the impact of illness, extended care placement, or end-of-life planning.
Provide psychosocial assessment, counseling, and discharge planning for long term care.
Implement social work treatment plans and documentation consistent with insurance guidelines.
Work collaboratively with other members of the patient care team to ensure successful care management.
Required qualifications for this position include:
Master of Social Work degree from a school of social work accredited by the Council on Social Work Education
Current Washington State registration as an Agency Affiliated Counselor
Current Basic Life Support (BLS) certification
Current Cardiopulmonary Resuscitation (CPR) training and negative tuberculosis (TB) screen
One (1) year social work experience in a health care setting
Valid driver’s license and insured, reliable vehicle
Preferred qualifications for this position include:
Current Clinical Social Worker licensure
Certification by the Academy of Certified Social Workers
Two (2) to five (5) years of social work experience in a hospital, hospice or palliative care setting
