Providence Health & Services RN Outreach Case Manager in Portland, Oregon
Providence is calling an RN Outreach Case Manager to Providence Medical Group in Portland, OR.
This is an exciting opportunity for an RN with case management and/or home visit experience who is seeking an opportunity to take compassionate patient care to patients in their own locations in the community.
This position is based out of our Providence Office Park location at 4400 NE Halsey, but will travel by car throughout the Portland metro area a significant percentage of the week.
In this position, you will:
Provide case management and care coordination services for at-risk patients, including patients with complex medical and psychosocial issues.
Utilize population-based disease management approaches and application of clinical protocols that are integral to the program.
Be a core member of the patient centered medical home teams led by the physician/provider.
Assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality.
Assist providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient's functional status and independence.
Required qualifications for this position include:
Current state of Oregon Registered Nurse (RN) licensure and graduation from an accredited school of nursing (strongly preferred)
Current recognition by a national professional organization in case management must be obtained within 2 years of hire date
Demonstrated ability to work cooperatively in a multi-disciplinary team
3 to 5 years of clinical experience
This position calls for a high degree of car travel within the Portland metro area in your personal vehicle. A current driver's license and ability to drive within the Portland area on a daily basis is necessary for this position.
Preferred qualifications for this position include:
For RNs, graduation from a Bachelors of Science in Nursing (BSN) program
3 to 5 years of clinical experience in Home health, skilled nursing facility and/or community health nursing
Experience in the field of chemical dependency and mental health strongly preferred
