Providence Health & Services Medical Social Worker - On Call in Portland, Oregon
Providence is calling an on call Medical Social Worker (0.0 FTE) to our Care Management Department at Providence St Vincent Medical Center in Portland, OR.
Our Care Management Social Workers work with the multi-disciplinary teams on the medical floors by providing on going psycho-social assessments and discharge planning Monday - Friday 8am - 4:30pm. We are looking for on call Social Workers to be scheduled in advance to cover planned time off and potentially short notice requests for coverage, if available.
In this position, you will:
- Upon referral and based on case findings, provide clinical social work services in the areas of:
Education to patients, families, and significant others involved in their plan of care .
Comprehensive care management, discharge planning and continuing care services.
Referral to community resources.
Required qualifications for this position include:
Master’s degree from a school of social work accredited by the Council of Social Work Education
Licensed clinical social worker by the Oregon State Board of Clinical Social Workers - or - must obtain their LCSW within a specified time period of hire date as determined with manager.
Preferred qualifications for this position include:
1 year of social work experience in acute care setting
Experience in crisis intervention, solution-focused therapy, and motivational interviewing
Knowledge of Medicare, Medicaid, and commercial insurance benefits
Knowledge of local resources as well as state and federal programs
