Providence Health & Services Certified Clinical Care Coordinator - Providence Family Medicine Southeast in Portland, Oregon
Providence is calling a Certified Clinical Care Coordinator to Providence Medical Group Providence Family Medicine Southeast Eastport Plaza in Portland, OR. This position is 40 hours per week (1.0 FTE) and will work 5(8) hour days: Monday - Friday from 8:00 am to 5:00 pm. Candidates must have strong Medical Assistant experience and leadership skills. In this position you will:
Serve as the key liaison between the patient and their patient-centered medical home team, including providers, clinic staff, and other health care professionals.
Work to ensure safety, best practices, and high quality standards are maintained for the patient across the health care continuum.
Required qualifications for this position include:
High school diploma or equivalent
Completion of an accredited Medical Assistant program and certified through a nationally recognized Medical Assistant program such as AAMA, AMT, (RMA) or NCCT
Current American Heart Association Basic Life Support (AHA-BLS) certification for the Healthcare Provider
3 - 4 years of recent healthcare experience working as a Medical Assistant
Preferred qualifications for this position include:
Experience with referral coordination
Strong leadership experience
Excellent documentation skills
Bilingual candidates preferred
