Providence Health & Services Staff Pharmacist in Polson, Montana
Providence is calling a Pharmacist (Lakeshore Pharmacy, Day Shift, 0.6 FTE) to Providence St. Joseph Medical Center in Polson, Montana. In this position you will:
Fill medication orders to all inpatient and/oroutpatient areas.
Provide pharmacy anticoagulation services in anoutpatient setting.
Individualize anticoagulation therapy based onindications, concurrent disease states, lab data, concurrent drug treatment,and pharmacokinetics.
Provide patient-focused care by recommending,monitoring, and evaluating patient-specific pharmacotherapy.
Interview to ensure appropriate patient historydata is documented.
Gather information via chart review, patientinterviews, and patient's retail pharmacy.
Provide quality patient specific druginformation independently by utilizing appropriate resources.
Ensure prompt and accurate delivery ofmedications to patient, while maintaining confidentiality.
Review, process and check physician orders andpatient medication profiles.
Provide pharmaceutical and drug interactioninformation to the medical staff, hospital staff, and the public.
Have responsibility for recommending changes in apatient's drug therapy and capable in interpreting and evaluating physicianorders for the most cost effective, appropriate therapy.
Ensure the consistency of quality and standardsof practice.
Required qualifications for this position include:
Bachelor of Science from an accredited School of Pharmacy
Current license or eligible for license as a Registered Pharmacist in Montana State
Preferred qualifications for this position include:
Doctorate of Pharmacy (PharmD)
Completion of American Society of Health-System Pharmacists (ASHP) accredited residency program
Board Certified Pharmacotherapy Specialty
Prior hospital pharmacy experience
