Providence Health & Services Registered Nurse - Cancer Patient Navigator in Olympia, Washington
Providence is calling a Registered Nurse - Cancer Patient Navigator to Providence St Peter Hospital in Olympia, WA.
This position will provide direct clinical support, education and development, and case management for Palliative Care Services in the Inpatient hospital and Outpatient clinic or Facility setting. Collaborates with the Palliative Care Interdisciplinary Team, Nurse Navigators, Spiritual Care Department, Inpatient and Outpatient care providers in the provision of quality Palliative Care. The Palliative Care Nurse navigator assists the Palliative Care Service team in responding to and triaging and assigning referrals; educating/mentoring nurses and other clinicians and non-clinicians in evidence-based palliative care; assisting the Care Coordinator in discharge planning and/or entry into the hospice program; and functioning as a case manager for patients throughout the continuum of care. The Palliative Care Nurse Navigator may be required to travel to select sites in the Southwest Washington Region to provide care and program development.
In this position you will:
Contact all patients with abnormal findings or definitive diagnosis to ensure compliance and prompt follow up.
Meet with patients when diagnosed with cancer and escort as possible to consultation and surgery appointments.
Contact patients to schedule and reschedule missed appointments.
Serve as liaison between the primary physician, palliative care clinician and the patient.
Identify patients that need navigation and assistance with barriers such as lack of insurance, financial issues, need for personal support, language, and any other problems that may arise and initiates appropriate referrals
Retrieve medical records as needed.
Work collaboratively with MSW / Case manager to eliminate barriers for accessing care.
To support the needs of newly diagnosed patients in primarily Thurston and Lewis counties, secondarily Mason, Pacific and Grays Harbor counties as program grows; 50% regional travel may be required as such.
Develops curricula and teaches classes to ensure competency development of interdisciplinary team.
Required qualifications for this position include:
Three or more years of experience in a hospital acute care setting
WA State Registered Nursing license or ARRT and Cancer Navigator Certification
Current AHA Basic Life Support (AHA-BLS) certification for Health Care Providers issued by the American Heart Association
Extensive understanding of oncology patient care
Preferred qualifications for this position include:
Bachelor of Nursing Degree or equivalent experience as a Registered Nurse will be considered
Certified Breast Navigator
Oncology Certified Nurse (OCN)
Oncology experience, discharge planning or case management experience
