Providence Health & Services Registered Nurse Care Specialist - Monroe Clinic in Monroe, Washington
Providence is calling a Registered Nurse Care Specialist to Providence Medical Group in Monroe, WA.
This is a full time (Monday - Friday; 40 hours per week) and benefits eligible position.
We are seeking a compassionate and caring Registered Nurse (RN) to assist patients in gaining self efficacy/management skills, achieve optimum functional health status, and quality of life.
In this position you will:
Have responsibility for managing patient populations with chronic illness and/or multiple co-morbidities.
Use skilled assessment, education, collaboration and coordination of healthcare and community resources to perform role.
Assist patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes.
May perform other ambulatory care RN duties.
Required qualifications for this position include:
Graduate of an accredited school of nursing
Current Registered Nurse (RN) licensure in Washington State
Minimum 2 years RN experience
Current AHA (American Heart Assoc.) Basic Life Support certification for Healthcare Provider (must be AHA accredited)
Preferred qualifications for this position include:
Bachelor of Science Degree in Nursing (BSN)
Certification of diabetes education (CDE)
Experience managing complex patient needs with co-morbidities in an ambulatory setting
Experience with Medical Home Model
