Providence Health & Services Social Worker - Senior Psychiatric Unit in Milwaukie, Oregon


Providence is calling a full time Social Worker to our Senior Psychiatric Unit at Providence Milwaukie Hospital in Milwaukie, OR.

Are you a Social Worker with a passion for working with seniors and their families? As an integral part of the interdisciplinary team, the Social Worker provides complex transition planning, educating patients and families about mental health issues and working with a wide variety of community partners. The schedule for this position is currently Monday - Friday during the day. This position may require weekend coverage on occasion based on the unit's needs.

In this position, you will:

  • Under direct and continuous supervision, provide comprehensive biopsychosocial-spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.

  • Incorporate concepts and theories associated with life span development, neurobiology, and behavioral change.

  • Collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Interdisciplinary Team (IDT).

  • Perform duties in a manner that respects patient self-determination that reduces health disparities/barriers to care and adheres to the NASW code of ethics, Standards of Practice (SOP), and mission.


Required qualifications for this position include:

  • Master’s degree in Social Work from a School of Social Work that is accredited or in candidacy status by the Council of Social Work Education (CSWE).

  • Temporary Clinical Social Work Associate (CSWA) license, or proof of CSWA initiated within 6 months of hire

  • Must obtain Licensed Clinical Social Worker (LCSW) Board licensure within a 5 years from CSWA application, or date of hire as a CSWA

  • Documentation of an approved supervision plan from the Oregon Board

  • 1 year of case management and/or medical social work experience

  • 1 year of behavioral health crisis intervention experience

Preferred qualifications for this position include:

  • LCSW

  • 1 year of experience in acute care settings

  • Healthcare experience
