Providence Health & Services Certified Clinical Care Coordinator- Mercantile in Lake Oswego, Oregon


Providence Medical Group is calling a Certified Clinical Care Coordinator to the Mercantile Medical Plaza in Lake Oswego, OR.

Mercantile Medical Plaza is looking for an experienced Medical Assistant OR LPN who is seeking an opportunity to delegate, lead and support a team of providers and medical assistants with maintaining patient flow and a high level of patient satisfaction. This is a Monday-Friday 8:00am-5:00pm schedule.

In this position you will:

  • Serve as the key liaison between the patient and their patient-centered medical home team. The team includes providers, clinic staff, and other healthcare professionals.

  • Work to ensure safety, best practices, and high quality standards are maintained for the patient across the healthcare continuum.


Required qualifications for this position include:

  • High school diploma or equivalent

  • Completion of an accredited Medical Assistant (MA) program - and- Certified through a nationally recognized MA program such as the American Association of Medical Assistants (AAMA), American Medical Technologists (AMT), Registered Medical Assistant (RMA) or National Center for Competency Testing (NCCT)

  • OR - Graduation from an accredited LPN/LVN School of Nursing

  • 3 years of recent health care experience working as a MA or LPN

Preferred qualifications for this position include:

  • Some college
