Providence is calling a Temporary Transition Planner-Social Services to Providence Regional Medical Center Everett in Everett, WA.
We are seeking a Transition Planner to plan and coordinate activities related to the continuum of care. Develop and implement a plan of care designed to meet the patient’s hospital and post hospitalization needs while ensuring continuity of care. Be responsible for the collaboration with medical staff, patient, family, community agencies, or facilities involved in patient’s care, and the health care team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes. This is a Day Shift/ Part Time (24 hours a week) that will require working weekends on a rotational basis. Benefits are offered with this position.
The Transition Planner partners with patients, families and the health care team to address and advocate for their social, medical, and emotional needs. The Transition Planner is responsible for providing a full range of social worker services including but not limited to psychosocial assessments, mental health assessments, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The Transition Planner will partner with the patient and his/her support system, as well as interdisciplinary team, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of healthcare resources. The Transition Planner will also assume a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes. The Transition Planner is guided by the NASW Code of Ethics and the Providence Mission and Vision in his/her daily practice.
In this position you will:
Be accountable for following the guidelines of attendance, punctuality and overall dependability.
Be accountable for effective performance and follow-through of all assigned responsibilities and for completing responsibilities within designated (or agreed upon) time frames.
Ensure that assessments for discharge needs are documented in the patient's chart based on referrals, department policies, or patient/family requests.
Assess all patients and/or family members regarding medical, social or emotional/coping background using appropriate assessment techniques.
As requested, provide pre-admission/admission evaluation for patients and family members.
Create discharge plans which will include appropriate, effective and current community resources.
Develop a clear, measurable, and effective plan for intervention which takes into account patient age and diagnosis related issues.
Demonstrate knowledge of the impact of diagnosis on psychosocial issues.
Assist patients and families to identify their personal and interpersonal resources.
Provide for or arranges to assist patient and family in dealing with emotional, family, financial and/or other issues.
Required qualifications for this position include:
Master’s degree in Social Work, Psychology or Counseling from an accredited school.
Prior social services experience in a hospital, hospice, psych facility, palliative care or drug rehabilitation.
Current Agency Affiliated Counselor, Licensed Mental Health Counselor or Associate; Licensed Advanced Social Worker or Associate; or Licensed Independent Clinical Social Worker or Associate.
WA State DOH licensure.
Preferred qualifications for this position include:
Experience in an acute hospital setting providing social services and discharge planning.
Certification or accreditation in Case Management.
