Providence is calling a Pharmacist to Providence Regional Medical Center Everett in Everett, WA.
Perform professional pharmaceutical dispensing functions by providing prescribed medications, drugs, and other pharmaceuticals according to standards and practices for patients in all age groups including neonatal, pediatric, adolescent, adult and geriatric. Promote safe, cost-effective, rational drug use and provide expert pharmaceutical consultation to other healthcare colleagues. Using Pharmacy & Therapeutics Committee approved protocols, prescribe narrow therapeutic index medication and ensure appropriate follow-up. Performs all duties in a manner which promotes and supports the Core Values of the organization .
In this position you will:
Demonstrates age specific patient care considerations for the population served
Participates in the development, implementation and monitoring of a pharmaceutical care plan to reach a desired outcome for each patient
Promotes cost-effective guidelines and individualized drug therapy
Follows written protocols within the Washington State Department of Health regulations to determine appropriate individual therapy
Minimizes or prevents the incidences and severity of adverse drug effects
Provides oversight and direction to staff
Provides drug information and education (written or oral) to patients and other customers
Maintains confidentiality of records or medical center information at all times
Required qualifications for this position include:
Doctorate of Pharmacy degree from an accredited school of Pharmacy
Completed ASHP-accredited PGY-1 Residency Program or 3 years of pharmacist experience.
Licensed as a Registered Pharmacist in the State of Washington.
ACLS certification must be obtained within one year of hire.
