Providence Health & Services RN Primary Care Transitional Care Center ($2,500 bonus and relocation) in Anchorage, Alaska
Providence is calling a Primary Care Registered Nurse to the Providence Transitional Care Center in Anchorage, AK.
The Primary Care Nurse will be in charge of the resident care for a specific group of residents, to include, supervision of team members, coordinating referrals to other members of the interdisciplinary healthcare team, families and physicians.
In this position you will:
Serve as the point of contact for resident information.
Assist with clinical interventions and resident activities of daily living.
Administer medications and treatments and provide direct licensed nursing care for a specific group of residents on a given shift.
Deliver report to the C.N.A.s and gain a return report from the C.N.A.s during the shift.
Notify the physician and/or nurse practitioner of resident changes of condition and receive orders thereafter.
Collaborate with all other healthcare professionals as needed for service for appropriate residents.
Required qualifications for this position include:
Graduate of an accredited/approved school of nursing.
Current Alaska Registered Nurse license.
Proficiency in medication administration.
Ability to become proficient in IV therapy within scope of practice according to licensure.
Ability to clearly articulate and communicate ideas verbally and in writing.
Preferred qualifications for this position include:
Six (6) months nursing experience in a health care facility in acute care, long term care, or a rehabilitation center.

The Transitional Care Center has a total of 52 beds with a variety of health rehabilitation issues (ages 20 and up). This is an exciting, busy unit with constant change, many admits, discharges.
