Providence Health & Services Coordinator - Transitions Program (Part Time) in Anchorage, Alaska
Providence is calling a Coordinator - Transition Program to Providence Home Health Care and Hospice in Anchorage, AK.
The Coordinator – Transition Program will develop and maintain the Providence In-Home Services Transitions Program.
In this position you will:
Develop and maintain the framework needed to serve pre-hospice clients and families in the community. Identify opportunities for volunteer services for these clients. Coordinate with Providence In-Home Services staff and other community programs to identify needs-based service opportunities and to ensure that needed community services are offered to clients as appropriate.
Maintain collaborative relationships and coordinate with other PH&SA departments and a wide variety of other community programs to enhance and expand service opportunities for the population being served by this program.
Communicate regularly with the clients, family, community programs, and appropriate community providers involved in the care of each individual client.
Maintain orderly, timely, legible, and accurate documentation of all client, family, provider, and community services interactions, all assessments of needs, and all follow up provided in response to identified needs.
Recruit and place volunteers in aptitude/skill level/interest appropriate service positions within the Transitions Program framework. Acts as an on-going resource for Transitions Program volunteers by providing guidance, support and developmental feedback where appropriate. Counsels and terminates volunteers when necessary. Develop and oversee volunteer leader opportunities where appropriate. Ensure annual volunteer health & safety requirements are met in a timely manner. Develop, administer, and provide analysis for annual volunteer evaluations.. Acts as a resource for other PH&SA volunteer programs as requested.
Coordinate all celebratory and recognition events for Transitions Program volunteers (i.e. Volunteer Week Celebrations).
Monitor program activities for quality and excellence. Track and communicate program activity levels and outcomes to appropriate audiences (leadership staff, internal and external publics). Identify opportunities for improvement and/or growth.
Required qualifications for this position include:
Bachelor’s degree in healthcare, human services, or related field (four years of professional experience in related positions may be considered in lieu of a degree.)
Excellent verbal and written communication skills.
Confidence in public speaking.
Strong organizational skills.
Ability to work as a team member and to interact in a positive manner with others.
Reliable transportation and automobile insurance according to Alaska State Motor Vehicle laws.
Preferred qualifications for this position include:
Two (2) to Four (4) years of experience in program coordination, volunteer management, community services coordination, and/or human resource development.
Strong customer service and interpersonal communication skills.
Demonstrated competency in providing education and information to the general public and healthcare professionals.
